Does inbreeding always lead to birth defects/deformation/mental problems?

Does inbreeding always lead to birth defects/deformation/mental problems in the child? Is there a possibility that the child does not acquire defects/deformation/mental problems, but instead something completely opposite (for example, very high intelligence)? If not genius, can the child be at least normal?
